Any comments made by the nurse in the intermediate screen before escalation, is now logged in the care team chat as a message from the nurse. 3. When integration is enabled for any unit, we have introduced a 'Sync Now' option, to perform a manual sync with the EMR at any point in time.

Entering height is now introduced as a step in CKD onboarding. In the Event to response configuration, for each of the events, the Role of the Care Team Member can be assigned. A new question is Included in the Check-in screen to track medication adherence for a patient on a daily basis. Now, Patient Tracker & User Feedback data are available in RAD. Now the Nurse can add their availability in the Profile section for the Hour-to-Hour configuration. The Care Team members can choose from a list of pre-defined responses to send to the patients.

Description of Reya Diabetes Remote Care

Reya Diabetes Home Monitoring helps nurses and doctors manage their patients on SMBG (Self Monitoring of Blood Glucose) remotely. Eligible patients are onboarded by a nurse on the smartphone app. They use the Patient App to add sugar readings. Nurses are immediately alerted of high or low sugar readings, allowing quick followup with patients. Doctors generate digital prescriptions for the patient remotely. The system enables care continuity and prompt feedback loops between the patient and their care team. Overall, this leads to better patient experience, and improved quality of care. This App has been integrated with Apple Health App to fetch Walking + Running distance data.
