## What is ABCD家泉戶曉?

此社會資本計劃由社區投資共享基金資助，香港中文大學醫學院那打素護理學院負責推行。

本計劃運用「社、校、醫」跨界別協作模式，以健康為切入點，希望透過一系列社區健康教育活動，提升水泉澳邨居民的健康意識。本計劃亦積極推動區內居民組織義工團隊，攜手建立鄰里互助網絡，把水泉澳邨打造為居家安老的社區。

APP內容包括水泉澳邨的社區資訊以及我們計劃舉辦的各項健康活動資訊，歡迎大家下載使用，多參與我們提供的健康活動或加入我們的「泉心護你」義工團隊，期望見到大家！
