## What is AblateCare?

AblateCare supports clinician-supervised follow-up after a cardiac ablation.

Use AblateCare to:

• Complete a structured daily recovery check-in
• Share symptoms and medication status with your care team
• Send and receive non-urgent care-team messages
• Connect Apple Health on the iPhone paired with your Apple Watch
• Share permitted heart rate, HRV, activity, workout, ECG, and rhythm-event context
• See when health information last synced

Your care team controls patient access. A linking code from the AblateCare patient website securely connects the iPhone app to your approved account.

Apple Health and wearable information supports clinical review. AblateCare does not diagnose arrhythmias, replace clinician judgment, or provide continuous or emergency monitoring.

If you have chest pain, fainting, stroke symptoms, severe shortness of breath, major bleeding, or another emergency, call emergency services or follow your emergency plan immediately.
