## What is aCare Home?

aCareHome is a shared care platform for Patients and Caregivers.

Patients can easily record daily care information, including medication intake and care notes. Caregivers can connect to a patient by scanning a unique QR code to view shared records, medication logs, and care history.

The app is designed for personal and home care coordination to help families and care teams improve communication and continuity of daily care.

aCareHome does not provide medical advice, diagnosis, or treatment. It is not a substitute for licensed clinical services or professional medical judgment.
