## What is Access Connected Care?

Access Connected Care or “ACC” connects patients with medical providers virtually for the medical management needed to create patient-centered whole health care. ACC incorporates two way communication between patient and medical support teams. The included ACC healthcare feed provides access to information on the latest medical and healthy lifestyle updates.
