First, it should be able to run in the background. You have to confirm the alarm, and be careful not to close the app or the phone, or it won't sound in the am. I've overslept because of this once, and learned my lesson. However, leaving it on all night runs the phone battery down, so I've overslept that way too. The third (and fourth and fifth) time I overslept was because once the message is finished, it doesn't repeat.
