TREATMENT TIMELINE
One record per chronic treatment. Every PA request linked to its treatment, building a multi-year history that survives the next renewal, the next denial, the next change of plan.

Every denial classified - Medical Necessity, Step Therapy, Experimental, Out-of-Network, Documentation Incomplete, and more. Verbatim reason saved alongside, so the pattern across denials becomes visible.

Internal Level 1, Level 2, External Review (IRO), State Commissioner, DOL EBSA, ERISA litigation - each with its own deadline math.

STEP THERAPY DOCUMENTATION

DEADLINE ENGINE
Standard PA decision (7 days under CMS-0057-F). Expedited (24-72 hours). Internal appeal filing (180 days). External review (4 months). Per-state ERISA differences.

COMMUNICATION LOG
Every phone call, reference number, promise made. Date, duration, contact name, your contemporaneous summary.
