It is a fun app but you can't use with voiceover, the built-in screen reader. It will be nice if the developer adds Accessibility to the app. There are only buttons and it should be very easy to do.

I always get a lot of laughs while talking on the phone and have the big button box open on my iPad. It is quite perturbing to some of the people that I’m not to friendly with and they cut the call short, which I don’t mind.  Telemarketers hang up right away.   Both big box apps are great.
