### 4/5 — Love the simplicity of this app...BUT...

...it would get a 5th star if I could put in profile info.  When I email this to my Doc, and they print it out...and it is sitting on the printer...and a few other things hit the printer, they will have no clue who this is for.  That seems simple enough to just add profile info...Name, DOB, address maybe, etc.  Otherwise, simple and easy to use.  OH...love the iCloud feature.  People forget we have multiple devices.
