## What is Capnolog?

Capnolog is a case log for anesthesiology and surgical residents that fills ACGME's Case Log form on your iPhone.

• Log a case in a few taps, from a template or your last case.
• See each ACGME category next to the minimum, with your pace to graduation.
• File on ACGME from your iPhone: Capnolog fills the form, and you press Submit.
• Plan tomorrow's cases with each attending's preferences on the plan.
• Catch up from a pasted case list, a CSV or a PDF.
• Export your whole log to CSV or PDF at any time.
• Medical students in any specialty track interviews, letters and the draft order.
• Your log records the kind of case, never whose.

FILE ON ACGME FROM YOUR IPHONE
ACGME's Case Log page opens inside Capnolog. Sign in on ACGME's own page, and Capnolog fills ACGME's form from your log. You check the form and press Submit yourself. On a computer, the Capnolog extension for Chrome fills the same form. Capnolog never presses Submit, never sees your ACGME password, and no Capnolog server ever touches acgme.org. Filing past the first 50 cases is part of Capnolog Premium. Capnolog is independent and not affiliated with or endorsed by the ACGME.

PLAN TOMORROW'S CASES
Build tomorrow's list the night before. Each plan shows your attending's preferences, and the plan fills in the case log after the case. Send a plan as a PDF or as text; the shared copy leaves out patient details by default. Case planning is free for 30 days from your first saved plan.

ROTATION-AWARE LOGGING (PREMIUM, ANESTHESIA)
Enter your rotation schedule, and Premium learns each rotation from your own cases. The case form puts your rotation's details first, and the rotation's usual technique, site, and attending fill in with one tap.

YOUR NOTES, FROM APPLE NOTES
Keep attending preferences, checklists, and photos in notes. Bring your notes over from Apple Notes and review each note before saving.

NEUROSURGERY
Residents at any ACGME program can join. Say the operation, type your own shorthand, or start from a template. Insights counts your cases by primary code and training year, and imported notes land in the neurosurgery note template.

MEDICAL STUDENTS IN ANY SPECIALTY
Track each interview with the date, time, track and thank-you note, keep a Letters list, and build your draft order. The ERAS summary works without a case log, and on Match Day Capnolog saves where you matched.

CATCH UP FROM A LIST
Paste a case list, or import a CSV or PDF you exported. Capnolog reads the columns, skips the cases you already have, and adds the rest. Nothing pasted leaves the phone.

PRIVATE BY DESIGN
The case log records the kind of case, never whose. Your cases stay on your iPhone, and a de-identified copy syncs to Capnolog by default; turning off Case sync in Settings deletes that copy. iCloud Backup uses your own iCloud, encrypted so only your devices can read it. Medical record and case-ID fields are masked to their last four characters before they are stored.

CAPNOLOG PREMIUM (OPTIONAL)
Logging, progress, export, the paste import, and the calendar schedule import are free. Premium adds case planning after the first 30 days, rotation-aware logging in anesthesia, unlimited templates and attending notes, ACGME Case Log filing past the first 50 cases on the phone and in Chrome, case lists, and remembered import formats. Premium is an auto-renewing subscription, yearly or monthly. Payment is charged to your Apple Account at confirmation and renews automatically unless cancelled at least 24 hours before the end of the current period. Manage or cancel in Settings › Apple Account › Subscriptions.

### 5/5 — Much easier way to log cases

*2026-10-17*

This app has made case logging so much easier. With just the press of a couple buttons you can add cases to your personal logs within seconds instead of having to go back and remember what you did after the fact. It also gives you real time feedback on how you’re progressing so you can focus on targeting cases you may be behind in. Definitely worthwhile

**Developer response:** Thank you for taking the time to review our app. Logging a case in the OR, while the details are fresh, is the main goal of Capnolog. We are glad the progress view helps you see which case types need attention before the end of the year. If you ever find a step that slows you down, please send us a note from Settings so we can look at it.

### 5/5 — Quick and Easy

*2026-10-17*

I can log a basic case in 10s, big one in 15. Can log 13 endo or ECT cases in less than 5 minutes in the OR, same-day. No sitting down at the end of the week at home (on my own time!). All the ACGME requirements are tracked in the app. Regional pain stuff included. The creator is quick to add components that are useful like case notes

**Developer response:** Thank you for the kind words and for sharing your times. Thirteen cases in under five minutes, logged the same day, is exactly what we hoped residents could do. Logging in the OR should give you back your evenings. Please keep sending ideas like case notes, because suggestions from residents decide what we build next.

### 5/5 — Way easier and faster method

*2026-10-12*

After using this app for a couple weeks, I have several conclusions. This app is way more intuitive and easier to use than the ACGME website. I like that it tells me my total counts compared to the ACGME targets. It also predicts the counts by graduation based on frequency of previous logs. It’s easy to sync to ACGME, and it automatically updates when a new attending is added to the options from ACGME. As long as you follow the instructions for the initial sync, the app and ACGME will mirror and update numbers. I like that it will also tell me what my assignment is for tomorrow and I can directly take notes in the app. Overall m, this app functions as a one stop shop for case logging and planning.

**Developer response:** Thank you for the kind review. We built Capnolog so logging takes less time than the ACGME website. We are glad the target comparison and the graduation forecast help you see where you stand. Getting the first sync right matters most, and after that step Capnolog and ACGME stay matched on their own. We put tomorrow’s assignment and your notes next to the case log so you can plan in the same place. Thank you for using Capnolog in your training. We can't wait to bring what you have experienced to all residents and programs nationwide.

## Version history (last 5 releases)

### 1.5.3 — 2026-10-19

Onboarding now asks for your program and your training year before you start. On iOS 26, Back returns from every screen opened from Log. After onboarding, Log a case opens the case form on the first tap.

### 1.5.2 — 2026-10-17

The first tap on Log case scrolls to the detail the case still needs. A second tap logs the case unfinished, and the case waits under To finish, where a tap opens the case on that field.

Neurosurgery residents can export their case record. The CSV is free, and the two record PDFs come with Premium.

An attending saved under a nickname shows the name ACGME uses beneath the nickname.

Every case reads Filed on ACGME or Not filed on ACGME.

This release also fixes several bugs.

### 1.5.1 — 2026-10-16

Medical students in any specialty can track the application season. Pick your specialty in Settings, under Applying to, and program search puts your specialty first.

Each interview keeps its date, start time, in person or virtual, city, track and thank-you note. The season notebook has a Letters list, and the ERAS summary works without a case log. On Match Day, a student without a case log saves where they matched.

Oral and maxillofacial surgery residents in the pilot program can log each case in the fields RSL asks for, and Progress shows CODA's minimums.

Onboarding lists every specialty's programs, so a resident in a specialty Capnolog does not log yet can pick a program before joining the waitlist.

Residents in any specialty can type in a rotation schedule or import one from the program's calendar.

When an attending is not on your roster, the Other field offers the names you typed on earlier cases.

Fixes: cases logged for a month without a day count toward your totals in Insights, and never toward a streak.

### 1.5.0 — 2026-10-15

Sign in with Apple works on iOS 27.

• A case you logged in Capnolog that is not on ACGME says so on the case, in All cases and after an ACGME read. When you file from this iPhone, File sits beside it.
• Imported from ACGME shows the cases ACGME already holds as one card. See all opens the list.
• A ticked checklist item slides to the bottom of the list, as in Apple Notes. One Undo puts the item back. Turn the sort off in Appearance & units.
• A note's More button opens the standard iPhone menu.
• A sign-in code copied from Mail pastes into the code boxes again.
• On a single training year, the Progress tab says which year it shows and counts the cases you logged in Capnolog that year. Graduation minimums and your imported ACGME history count on Cumulative.
• The Log tab shows how many UAA cases are waiting to be logged, and a UAA case on the Plan tab leads with Log.
• Two weeks before the Chrome extension link ends, the Log tab says the day and how to renew the link. Once the link ends, the card offers to file from this iPhone.
• When your sign-in has ended on this iPhone, the app asks you to sign in again.
• A tap on a day in the date calendar sets the date and closes the calendar, and a tap anywhere outside the calendar closes it too.
• The save bar is Log case and Finish later again. Filing on ACGME from this iPhone starts from the card on the Log tab.
• Repeat last copies the case you logged last, on the day you are catching up.
• The ACGME card on the Log tab says how many cases are not yet on ACGME.

### 1.4.0 — 2026-10-13

Bring your notes over from Apple Notes. Tap Import on the Notes tab, and review each note before saving. In neurosurgery, the import places each note into the neurosurgery note template, and on an iPhone with Apple Intelligence that sorting runs on the phone itself.
• Photos in your notes now sync through your own iCloud, sealed on this iPhone first.
• A photo over 25 MB stays on this iPhone, and the note says so.

Case planning is free for 30 days from your first saved plan, and the planning screen shows the date. After that, planning is part of Capnolog Premium. Importing your rotation schedule from a calendar is now free.

Also in this version:
• A case counts toward your ACGME totals once the case has a hospital and an attending, because ACGME's form requires both. A notice on the Log tab says how many cases need them and opens the list.
• The ACGME page in Settings is now titled ACGME. The browser extension is under Settings, ACGME, Browser extension.
• When a case is ready to file, Save sits beside Save and file on ACGME.
• Once you file through Capnolog, a weekly reminder names the cases you logged three or more days ago that are not yet filed on ACGME.
• In anesthesia, All cases filters by surgical specialty.
• A plan shared without patient details can include your notes when you switch on Include my notes.
