## What is Carisk?

Welcome to CareLink 

CareLink helps you stay connected with your Carisk care team throughout your recovery journey. The app makes it easier to communicate with your healthcare support team, follow your care plan, and track your progress between appointments. 

With CareLink, you can: 
-Message your care team securely 
-View personalized care plans and educational resources 
-Track symptoms, pain levels, and wellness progress 
-Connect approved health devices to share health information with your care team 
-Receive reminders and updates to help you stay on track 

Remote Monitoring & Connected Devices

Depending on your care plan, you may be asked to use connected health devices such as: 
-Blood pressure monitors 
-Thermometers 
-Weight scales 
-Activity trackers 

Some devices connect through Bluetooth, while others send information automatically through a cellular connection. CareLink may also connect with Apple Health to help simplify tracking your health information. 

Personalized Care Plans 

Your CareLink experience is tailored to your individual needs. Your care team may ask you to: 
-Record vital signs 
-Complete wellness surveys 
-Track symptoms or pain levels 
-Review educational materials 

Information you share through CareLink helps your care team monitor your progress and provide support when needed. 

Health Progress Dashboard 
CareLink gives you access to a personalized dashboard where you can view health trends and track progress based on your care plan. 

Secure Messaging
You can securely message your care team through the app for support, follow-up questions, and care coordination between appointments. 

Appointment Scheduling 
Please continue scheduling appointments directly with your healthcare providers unless you receive different instructions through CareLink.
