使用肾尚CDM，用户可直接获得其医院主管医护团队的辅导，正确开展用户自我慢病管理。使用肾尚CDM，用户可记录和跟踪对于慢性病患者非常重要的健康管理行为，例如日常锻炼、饮食、血压、血糖或用药记录等。 用户记录的数据将安全地传递给用户所属的专科主治医师及其医护团队，他们将结合用户的其它体检记录、门诊记录等医疗数据，对用户的日常数据进行分析评估，并向用户提出最合适其自身状况的健康管理活动内容、要求及其注意事项等。
