Welcome to the ChartEasy™ Patient Portal, where as a patient you may directly view, download, and transmit your health records! Theoria Medical specializes in helping skilled nursing facility and primary care teams optimize patient care. We developed a proprietary electronic health records platform called ChartEasy™ to facilitate the most accurate and efficient documentation of your care by our providers.

View Lab Results and Vital Signs
Enjoy real time access to your lab results and vital signs to stay well informed of your health statistics.

View Medication
Your personal access to the portal provides you with a list of your current medications.

Access Chronic Care Management Plan
If you are enrolled in Theoria Medical’s Chronic Care Management Program (CCM) you and your caregivers may view and print your individualized care plan that has been specifically created by our care tram to help manage your chronic conditions.

Download and Transmit Your Records
You will enjoy complete and secure access to your personal records while also being able to allow any other authorized representatives you choose to access your records. Also, you can simply transmit your information to whomever you want, including other healthcare providers.
