I'm an EM physician and have been looking for an AI scribe that is customizable for our notes rather than soap notes.  It took some time to build a template, but now it works amazingly well writing all my notes.  Incredible product that keeps getting better.

Looks like the developers listened and introduced an unlimited notes option for $100. 
Overall it’s a great app. Can be used to do the notes on the go. I don’t have to try to remember small details that I can already put in a precursor notes.

I am blown away by this dictation app! I downloaded it because dragon is windows based and I wanted an app centered around phone use . When they say AI they mean it , the best way to appreciate the wow effect of what this entails is to trial it out . I am still on a trial (planning to get the paid version ) and when I opened up new templates , all I was expecting was straight transcription of my spoken words . It takes the words and also organizes , restructures and documents them into complete sentences . One really just has to see by trying as it does no justice to explain over a review  . Thank you makers for this app!! Every couple years I’ve searched for an app for just basic dictation and this one finally arrived far exceeding what I ever thought technology could come up with ! Seriously this has got to set the standard for other apps to come (which at the current rate won’t be a long time )…

I work in a very busy ER and I’ve tried several AI charting systems, including all medics, freed, and by far chart note has blown it away. It has a copilot option that the other AI system don’t have at all, which helps you look up certain things that you may have forgotten quickly.I love it. Does not capture when the patient rambles on and on about nonsense that has nothing to do about their medical visit. 100% recommend this.

Using the AI scribe was as close to magic as I have ever seen. I was able to listen to my patient and engage with them while not worrying about remembering to note down every piece of information. I leave the room and the note is nearly perfect with just a few edits.

After trying out the free 20 minutes and giving it a day of work, I found it to be too helpful in getting the notes out quicker that I just subscribed to their Professional (unlimited voice dictation) service. It’s a good (maybe even equivalent) alternative to Dragon Speech when your EHR doesn’t support voice dictation.

The concept is a great idea. I like how it has the option of free text - allows for flexibility in an academic medicine setting. However, the transcription quality is extremely poor and frequently misses common words (for example, it cannot recognize the word "what's"). The transcription of medical words is decent at best, but even that is often incorrect (it could not recognize or learn TMP-SMX, etc). It’s frustrating not only because the frequency of errors, but because I cannot correct them as I speak. I have to turn the microphone off, wait for it to calibrate, then scroll to the part I want to edit. 
Additionally, the program has poor interfacing between the app and website—if you have been transcribing in the app, you cannot use the website or it will never allow you to use the app for that note again. Finally, I emailed customer support to ask for guidance on the software’s apparent inability to recognize common words, and they have not gotten back to me. Overall, I am disappointed in this service. It would be easier to accept if there weren’t better transcription alternatives, but I can literally rapidly mumble medical terms into ChatGPT with extremely high accuracy (obviously not HIPAA compliant though). With chartnote, it still fails when I speak slowly and over-enunciate, and I have no accent. Even siri is significantly more accurate unfortunately.

Like a kid playing a game, you have to buy credits to use ai. There are other apps with unlimited notes/use of ai for about $100 subscription fee per month. If you have a high volume practice, this is not worth it. Eg if you see 600 pts and want to use ai, you have to shell out around $100.

Absolutely love this app, but they have recently forced everyone to do 2FA and it has been very buggy, clunky and is incredibly tedious.  This really needs to be made optional for those who don’t want to use it.

We rolled out MFA with security in mind, but it became clear that the experience wasn’t meeting expectations for speed and reliability in day-to-day clinical use. Based directly on feedback like yours, MFA is now optional, and we’ve cleaned up the flow to reduce friction.

It’s completely changed how I look at medicine and and my long-term outlook in this career. i’m not kidding when I say it has made me a better husband and parent because of 8+ hour more I get to spend with them each week. The developers have heard the feedback of their users and have finally made MFA optional!  I've used a ton of ai scribes over the years and there are none that match the customizability and accuracy of chartnote.  Thanks for making a great product!!

I have been using Doximity for my dictation. I have found that it does not know common medication names, and can’t always separate my voice from the patient’s. It also includes a lot of “noise” from the conversation. Chart Note removes all unnecessary details and helps the note come together in an accurate concise form. I am very pleased and have invested for another year.

Records are not my strong suit and I let them pile up. Because of this app, the quality of my records have been bettter than Ever. It captures so much detail and somehow knows what is relevant audio and what is background noise. I’m using it for every aspect of record writing, and it’s an incredibly worthwhile investment.

I can’t believe I didn’t find out about this until October 2024! Since I was a medical student in the 1990s, I’ve been looking for faster and easier ways to write notes on my hospital and clinic patients. In fact, note-writing has always been my least favorite part of medical practice. Keeping up with my charting has been an albatross around my neck, a constant burden always weighing down on me.

NO LONGER, thanks to Chartnote! Now, I place my iPhone down on the desk, press the AI Scribe button, and talk to my patient. I make sure to review everything that’s going on with the patient, what meds they’re taking, any recent tests (and results) they’ve had, and I talk through my physical exam saying things like, “Well, your lungs sound clear, your heart sounds normal, etc.” Amazingly, this app figures everything else out. It fills in the gaps (ie, a normal heart is translated to “regular rate and rhythm, normal S1 and S2 heart sounds, no murmurs, ribs, or gallops”), and pieces together a chronological HPI, even when the patient doesn’t give it to me chronologically! It is absolutely OUTSTANDING! 

The funny thing is, it feels almost like there’s an actual person on the other end of my iPhone, listening to my visit with the patient, and then writing the note for me!! I almost feel guilty making this virtual person do my chatting work!! Um… ALMOST…but not quite. 😉

People who are still writing their own notes are crazy…or more likely, just unaware that this kind of technology actually exists. If you’re any kind of clinician and you hate writing notes (like most of us), then get with the age of AI and download Chartnote Mobile ASAP! It has improved my work life more than any other new technology in the past 30 years! The future is here now! I’m sorry for the cliché, but this time, it’s actually the truth.
