### 5/5 — Thank you

*2020-04-05, version 1.0*

This is a great app for healthcare workers and the public alike to track their symptoms and perform self monitoring.

I like the simplicity, that you don’t have to set up an account, you can just get started screening.

I like the link to the cdc and the ability to transmit the data to your pcp. (Fortunately, I haven’t had to use that feature, so I don’t know how it works.)

One suggestion:
It would be very helpful if the demographic information you enter to start the screening process could be stored so you dint have to enter it every time.

### 1/5 — Absolutely useless

*2020-04-05, version 1.0*

Lol... I put headache and it told me “urgent” and advised me to go see medical professional😂😂 what a joke

### 5/5 — Easy screener with specific recommendations

*2020-04-05, version 1.0*

Screener that was east to take and had specific recommendations on how to proceed.

### 5/5 — Reliable

*2020-04-04, version 1.0*

Great app! This app is brought to you by experts in the field of infectious disease and the federal quarantine and bio containment unit. Can’t get better than that.

### 5/5 — Great resource

*2020-04-04, version 1.0*

This will help thousands who just want some information about their symptoms without having to go see a doctor.

### 5/5 — Love this app!!

### 1/5 — Consult Your Physician...

Sore throat? Cough? Hung over? Stubbed your toe? Go see your physician is what it tells you no matter what. Pointless.
