ChronicOS helps adults organize and track multiple chronic conditions in one place—without replacing your doctor.

Whether you manage diabetes, hypertension, kidney disease, or several conditions together, ChronicOS keeps symptoms, medications, labs, appointments, and visit notes connected so you can prepare for care and spot patterns between visits.
