## What is Circle Integrated Care?

This is the Circle Integrated Care app - for our patients or for those wishing to refer themselves to us.

This app allows you to access a range of information, inc;luding: any home exercises prescribed to you; information about your condition and all relevant contact details. This will help to support your care.

Please note that you will only be able to self-refer if you live in an area where we provide care. Please visit cic.circlehealthgroup.co.uk/patients/ to check eligibility.
