## What is CKD LifePlan?

There’s a lot to learn about chronic kidney disease (CKD) and the treatment options if your kidneys fail. CKD LifePlan was designed to help you learn about how CKD affects your body and understand your treatment options. 

You are the most important member of your care team. We're here to help you learn and ask questions, understand which treatment's best for you, and participate in your treatment plan.

CKD LifePlan is free to use for patients. All information in the app is developed in partnership with patients, renal nurses, and nephrologists.
