### 1.1 — 2026-09-03

This release applies a full clinical content review of every dose, energy, physiologic target, time window and citation in the app. The content below reflects the 2025 AHA Guidelines for CPR and ECC, ATLS 11, and current specialty guidelines.

POST-CARDIAC ARREST CARE
- Oxygenation: titrate FiO2 to the lowest level for an SpO2 target of 90-98 percent, avoiding hyperoxia. The module pearls carry the same figure.
- Temperature control: select and maintain a constant target temperature in the 32-37.5 degrees C range for at least 36 hours, then actively prevent fever.

CARDIAC ARREST AND DEFIBRILLATION
- Shock energy: use the manufacturer's recommended energy, commonly 120-200 J; if unknown, use the maximum available. Second and subsequent doses should be equivalent; higher doses may be considered. Monophasic: 360 J.

AMIODARONE IN ARREST
- The consolidated dose card now states the cumulative ceiling: first dose 300 mg IV/IO bolus, second dose 150 mg, maximum 450 mg cumulative in arrest.
- The dose notes spell out the regimen: one 300 mg dose followed by ONE 150 mg dose, 450 mg cumulative, with no third arrest bolus.
- The live code runner enforces that ceiling. The Amiodarone button logs the 300 mg and 150 mg doses, then disables itself with the label "Amiodarone - max 450 mg" and records that the cumulative maximum of 450 mg has been reached. Resetting a code clears the count. Note that a post-ROSC amiodarone infusion is a separate regimen and is not what this button records.

TRAUMA AND ENVIRONMENTAL EMERGENCIES
- Neurogenic shock and isolated spinal-cord injury: restore volume first, then norepinephrine, augmenting MAP to at least 75-80 mm Hg but not higher than 90-95 mm Hg, for 3-7 days, per the 2024 acute spinal-cord-injury hemodynamic clinical practice guideline.
- Frostbite: rewarm in circulating water heated to 37-39 degrees C, and assess for thrombolytic therapy with a regional referral center.

SOURCES, TERMS AND SCOPE
- The ATLS citation reads: Advanced Trauma Life Support (ATLS) Student Course Manual, 11th edition, American College of Surgeons, 2025.
- The Terms of Service source list and the disclaimer Scope paragraph both name all four bodies behind the content: AHA ACLS, ILCOR, ACS ATLS and ENA TNCC.
- The adult-dosing scope note reads: dosing shown is the ADULT standard, and pediatric cardiac-arrest resuscitation requires weight-based PALS dosing and is out of scope.

Every dose, energy and interval in this app still requires independent confirmation for the actual patient against current guidelines, local protocol and pharmacy resources.
