## What is Compwell?

This is a health and wellness application designed to assist the patient in tracking and managing their health and fitness goals.
This mobile app is used by Chronic Care Management(CCM) patients and physicians. Patients will record their vitals, physical activities, symptoms, and diet. Patients can view their care plan and call summary details. The Physician can view the care plan and call summary details of their patients.
