## What is Disability Passport?

Parents and carers often have to explain the same support needs again and again to schools, carers, family members and professionals.

Disability Passport gives them one place to keep that information up to date and create a PDF when they need to share it.

Create a clear, personalised profile for a child or adult. Record the information that matters to the individual, including:

• communication preferences and processing needs
• sensory needs and signs that support may be needed
• routines, preferences and reasonable adjustments
• strengths, interests and important people
• things that help and things to avoid
• allergies, health information, mobility, eating and drinking support
• emergency contacts and additional notes

Every field is optional, so the passport can reflect the person's actual needs. Update it directly on your iPhone or iPad whenever something changes.

Create and preview a one-page support profile, health appointment passport or quick support card. Generate a PDF to save, print or share with schools, carers, family members and professionals. The first PDF export is included, with a one-time in-app purchase available for unlimited exports.

Profiles and photographs are stored by the app and sync through your private iCloud account, helping keep them available when you move to a new iPhone or iPad.

Disability Passport can be useful for a broad range of disabilities and additional support needs, including autism. It is an organisational tool and does not provide medical advice, diagnosis or emergency assistance.
