According to my bank, EZCardInfo is online ONLY and does NOT have a corresponding app. I because highly suspicious of this app because they wanted far more detailed personal information than would even be required for card access online.

If you download this app all you will get is 

“You do not have permission to login mobile device.”

I cannot find permission settings for mobile on the website anywhere…
