## What is FreedomScribe?

FreedomScribe helps counselors document individual counseling sessions. With the client's logged, on-the-record consent, it records the session, transcribes it, and drafts a progress note in the counselor's chosen format (DAP, SOAP, and others) for the counselor to review, edit, and finalize.

The counselor is always the author. Nothing is final until the counselor reviews the draft against the session and approves it. FreedomScribe never files a note or submits anything on the counselor's behalf — the finalized note is exported for entry into the counselor's system of record.

Built for the realities of behavioral-health documentation:
• Consent is captured and logged before any recording can begin.
• The counselor reviews and attests to every note before it is final.
• Notes can be re-expressed in a different format while the original signed version is retained.
• Designed for confidentiality of sensitive treatment information end to end.

FreedomScribe requires an account. It is a tool for licensed and credentialed clinical staff, not a consumer medical product, and does not provide medical advice.

Licensed House of Freedom counselors can now document client sessions end to end — record
with the client's logged consent, review the AI-drafted note against the transcript, and
sign it.

Sign-in is required. FreedomScribe is for authorized House of Freedom clinical staff.

• Live microphone meter while recording, so you can see at a glance that the session is
  being captured — and a warning if no sound is reaching the microphone.

• Documentation Assistant for help drafting and tightening your own note language.

• Your sessions and notes now follow you across your own devices.

• ICD-10 and CPT suggestions on iPhone and iPad, drawn from the session transcript. You
  always choose the codes.

• Manage your client list: edit a name, archive a client you're no longer seeing, or
  remove one added by mistake.

• Reset your own password from the sign-in screen.

### 1.0.1 — 2026-08-07

• Suggested ICD-10 and CPT codes on the review screen, with the transcript passage supporting each one. You decide which to accept — nothing is submitted to billing.
• Reformat a finalized note into another format. The version you signed is retained.
• The note now records what the AI drafted alongside what you signed.
• Fixes: a crash when tapping the signature field, and a sign-in issue where iOS could alter typed passwords.
