## What is Haven Health?

This hospice app provides a convenient way for healthcare providers to refer patients to At Your Side and learn about our programs and services. Referrals submitted through the app will be processed for expedient follow up. All information received through the referral form complies with and will be processed according to HIPAA regulations.
