“HEALTH+“是专门社区或者乡镇等医疗机构的医生提供的App。App主要功能包括：
1、居民管理：显示乡镇或者社区内的居民，可直接进行在线签约服务;
2、签约申请：医生查看居民的签约申请，同意或者拒绝居民的签约;
3、居民咨询：提供医患互动的桥梁，医生回复居民咨询；
4、签约率统计：医生查看社区或者乡镇的签约率统计情况；
5、随访管理：医生为重点人群居民执行随访，填写随访记录表；
