I have been unable to properly connect Apple health to the app.  This is despite double checking that they are allowed to communicate with each other and disconnecting and reconnecting the app.

The first screen of this app contains a survey with drop down menus. The menus will not appear, which means I can’t finish the survey, which means I can’t access the app. So frustrating.
