## What is I?

新竹國泰醫院於2014年承接中央健保署“提升急性後期照護品質計劃”，主要針對因腦中風而產生失能的個案，包括肢體、認知、語言、吞嚥等障礙，甚至情緒憂鬱等問題，提供積極性的住院整合照護與復健，使其恢復功能。

出院之後的返家照護，個案仍然需要持續的復健治療，以維持身體的功能。

新竹國泰醫院急性後期照護醫療團隊，為提供給腦中風個案在出院後，能夠持續居家自我復健或在他人協助下持續復健治療，設計“I-Rehab復健小幫手“APP，分為物理、職能、語言及吞嚥等部分，並委由明新科技大學製作，供相關的個案使用。
