## What is 高醫腎好 iCKD?

由高雄醫學大學附設中和紀念醫院腎臟內科團隊設計與
執行，甫獲得經濟部之支持與補助。這是結合資、通訊
科技技術，如手機與網站，成為腎臟病患者與醫療團隊
間的醫病溝通管道。患者若有意願加入此服務者，須提
供其個人資料外，也須配合每日經手機上傳個人的血
壓、活動資料、飲食與用藥紀錄，作為未來在臨床診斷
分析及照護的參考依據。
主要功能：
1. 血壓監測資訊：將量測所得血壓數據，由手機上傳到
照護中心伺服器儲存，藉由長期紀錄與比對方式，供
照護團隊參考血壓對病情之影響。
2. 個人用藥資訊：紀錄個人相關用藥資料及分量、使用
時間，藉以強化按時用藥及安全用藥的重要性。
3. 個人衛教資訊：依據個人需求以簡單易懂的方式提供
最新照護知識，同時做好自我健康行為的轉變，藉以
維持腎功能不致於太快速惡化。
4. 關懷系統資訊：包含回診通知、門診醫囑錄音等訊
息。平日系統若未收到上傳的資料，將會適時(其頻
率可依需求自行設定)發出提醒簡訊。
5. 醫療資源搜尋資訊：提供所在地點的醫療院所搜尋功
能。
