## What is สมุดบันทึกสุขภาพ?

การพัฒนาระบบสมุดบันทึกสุขภาพบนโมบายแอปพลิเตชั่นเพื่อแก้ไขปัญหาที่เกิดและเพิ่มประสิทธิภาพในการใช้สมุดบันทึกสุขภาพที่เป็นหนังสือในปัจจุบัน ดังนี้
1. สมุดบันทึกสุขภาพมักเกิดการสูญหายบ่อยครั้งทำให้ข้อมูลที่ถูกบันทึกไว้สูญหายและไม่สามารถกู้คืนได้
2.เพื่อให้เกิดการแชร์ข้อมูลในสมุดบันทึกสุขภาพระหว่างหน่วยบริการสุขภาพ หน่วยงานกำกับนโยบาย
3.สร้างระบบสารสนเทศที่ทำให้ข้อมูลเป็นข้อมูลที่ตรงกัน (single source of truth) 
4.เพื่อให้ข้อมูลแก่หน่วยงานระดับนโยบายใช้วางแผนสุขภาพประชาชน
5.เพื่อให้ข้อมูลแก่หน่วยงานระดับชุมชน เช่น โรงพยาบาลส่งเสริมสุขภาพตำบลองค์การบริหารงานส่วนท้องถิ่นจังหวัดใช้ในการติดตามดูแลและให้บริการสุขภาพประชาชน
6.เจ้าของข้อมูล (ผู้สูงอายุ ผู้พิการ แม่และเด็ก)มีส่วนร่วมในการจัดการข้อมูลของตนเองได้
