## What is 醫關懷(團隊版)?

『醫關懷團隊版』是使用『醫關懷』服務的醫護成員所使用的APP。

APP提供個案關懷、關懷對話、關懷排程、關懷筆記、追蹤討論等關懷功能，當個案步出醫院身處家中或他處，團隊也能傳達貼心囑咐與遠端關懷給個案。

團隊版功能簡介:

「個案關懷」：依每個人的情況，提供個人專屬的衛生教育資訊。

「關懷對話」：隨時隨地即時的健康諮詢。

「關懷排程」：復原中各階段的注意事項，關懷鼓勵，回診時間等提醒。

「關懷筆記」：記錄復原歷程，提供後續的健康建議。

「追蹤討論」：團隊成員間追蹤討論個案狀況。
