慢病防控是在全民社区慢病管理的大背景下，由国家中医药管理局中医药科技发展中心、中国中医科学院中医基础理论研究所、南方医科大学第七附属医院等机构主导构建的线上慢病管理平台，集家医签约、健康评估、慢病档案、健康咨询、方案查询、慢病资讯等功能为一体。平台汇集全国慢病领域知名中西医专家团队，以提高患者个人健康能力为导向，为公众提供在线的中西医结合的医疗、慢病管理服务。
