## What is 永慈智联?

永慈智联隶属于翼依疾病管理平台，平台设计定位于将专家制定的慢病全过程防治体系全面落地与基层，建立慢病管理新的模式。平台本身支持多种慢病健康管理路径配置，目前正逐步实现由单一疾病管理过渡到五病同管（冠心病、高血压、糖尿病、脑卒中及慢性阻塞性肺疾病）。永慈智联定位于接收并执行平台派发的各类健康管理任务，降低医生负担，赋能医疗服务水平。
