## What is つたえる診察メモ?

つたえる診察メモは、症状をうまく言葉にできないときに、診察時に見せるメモを選択式で作り、受診後の記録も残せるアプリです。

主な症状、場所、いつから、つらさ、生活への影響、薬やアレルギー、医師に聞きたいことを選ぶだけで、受付や診察室で見せやすい文章にまとめます。

受診後は、病院名、診療科、医師からの説明、診断名や見立て、検査、薬、次回受診日、気をつけることを記録できます。

入力内容はサーバーへ送信されず、端末上でメモを作成・保存します。作成したメモや受診記録はコピーできます。

このアプリは診断、受診要否の判定、治療方針の提示を行いません。強い症状やいつもと違う様子がある場合は、119番または地域の救急相談窓口、医療機関へ相談してください。
