## What is iDAS28?

関節リウマチ（Rheumatoid Arthritis, RA）の診断や評価にはDAS28やACR/EULAR classification criteriaなどを使用します（＊）。項目選択や数値を入力するとその評価結果が出すアプリです。

DAS : Disease Activity Score　(疾患活動性スコア）
関節リウマチ（RA）患者の疾患活動性を評価するために 1980年代にNijmegenが開発された指数です。 日常診療で簡単に、患者の疾患活動性に関する貴重な情報を収集することになります。欧州リウマチ連盟(EULAR)の「response criteria」との組み合わせで、治療に対する反応の評価を行えます。
44関節の評価方法は煩雑なため、日常診療では28関節の評価を行う簡易的な「DAS28」が広く用いられています。

SDAI(Simple Disease Activity Index)
2010年11月に開催されたACRでは臨床的寛解評価のための指標として、DAS28に代わってこれを使うことが提唱されました。寛解基準は •	Boolean基準: 腫脹関節数、圧痛関節数、患者全般評価、CRP(mg/dL)のすべてが1以下 •	疾患活動性指標: SDAI≦3.3
CDAI(clinical disease activity index)
これも臨床評価に適しているとされ、寛解基準は •	Boolean基準: 腫脹関節数、圧痛関節数、患者全般評価のすべてが1以下 •	疾患活動性指標: CDAI≦2.8

ACR/EULAR新分類基準
中〜大関節： 肩、肘、膝、股関節、足首を含む
小関節　　：MCP,PIP,MTP2-5,1stIP,手首を含む
（注）DIP、1stCMC、1stMTPは除外
低値の陽性：基準値上限より大きく上限の３倍以内の値
高値の陽性：基準値の３倍より大きい値
ACR/EULAR新分類基準の妥当性について問題はないが、鑑別診断を適切に行うことが必要であり、本基準の使用にあたっては鑑別疾患難易度別リスト（表１）と問診表（図２）を併用することが望ましいとされています。
