For any clinician, this is the most useful app. I probably use it once or twice a day. 
The acid-base explanations are spot on and the differential diagnoses are complete. 
I use the EKG's as references for my medical students and I have my residents refer to it on rounds all the time.

Every doctor needs this in their pocket. Automates calculations that I typically needed to look up and provides great explanations.
