Diabetes Management platform to collect patient information using RPM, RTM, CCM, PCM, and BHI for outpatient clinic practice. Sharing information between primary care and Specialists.

Collaboration of Care between Primary care and specialist using an App based platform for patient Remote Physiologic Monitoring, Remote Therapeutic Monitoring, Principal Care Management, and Chronic Care Management. The platform also covers outpatient clinical practice for MDs, APRNs, Physical Therapists, Chiropractors, Pharmacists, Speech Therapists, Occupational Therapists, etc. Keep patient engagement ongoing, increase monthly revenue per patient, reduce medical events, and provide overall better health outcomes. Physiologic vital signs monitoring with combined Therapeutic Symptom Monitoring, Chronic Care Management, Principal Care management, and Behavioural Health integration. Collecting data remotely from patients using State-of-the-art remote 4G / Bluetooth Vital Signs wearable monitoring gives a comprehensive total remote health environment. Patients can monitor their Food intake, vital signs, blood glucose, weight, sleep, and physical activities.
