What is it? 

The Emergency Department Screener is based on the Assessment Urgency Algorithm embedded in the interRAI Contact Assessment Emergency Department module. This screener predicts the need for follow-up assessment and risk of poor outcomes. The Emergency Department Screener has 6 risk levels indicating need for referral or assessment, where 1 is “Least Urgent” and 6 is “Most Urgent”. 

What is the purpose? 

To help prioritize referrals for focused assessment and services in the emergency department, community, or inpatient unit. The results of the Emergency Department Screener are designed to inform, but not replace, best clinical judgment.Your final clinical decision should be based on all information available to you. 

Who is the target population? 

Persons aged 75 or older who are not in severe acute distress in the emergency department or primary care. Clinical judgment should be used to determine if younger persons would benefit from being screened. 

How do I use it? 

Follow the question prompts and use your best clinical judgment to arrive at each answer. The person is the main source of information. If available, informal helpers and medical records may be used to augment information provided by the person.

### 3/5 — Latest upgrade terrible!

The original version was far more user friendly on an iPhone. With this upgrade you must scroll down the screen to be able to find the "yes/no" options. Also the ability to "start over" at any point has gone, meaning you have to hit "back" multiple times to go to the beginning. Agh! Bring back the old layout  please!
