Jasper Care+ is a smart triage dashboard designed for clinicians, by clinicians to help track and monitor your patients. Care+ provides a hybrid approach to the collaboration among the interdisciplinary teams. Care+ also streamlines workflows for clinical teams and Jasper cancer care coaches to provide a never-before-seen, whole-person data view including clinical data and non-clinical data. These real-time data insights improve patient experience and outcomes by coordinating and navigating care to guide patients’ behavior and reduce acute events with patient risk stratification. Care+ also works as an organic engagement tool, helping members connect to their coaches and empowering them with data and information to provide the right care at the right time. Efficient and timely care coordination among teams to track the overall health and needs of each patient, in one view. Clinicians can quickly scan the green-yellow-orange and red dashboard to flag patients for rapid support while easily tracking members over time. The two steps outline the process: TRACK AND TRIGGER -- Tracking is done via our smart clinical dashboard and high-risk surveillance monitoring to surface any patients that deviate from “normal” or show signs of abnormality, to trigger alerts to the patient. TRIAGE AND ADDRESS -- Based on exception-based triggers, we can alert the patient and through our coaching sessions reach out to provide psychosocial support or advice to seek additional or emergency support.
