## What is Know My Heart (AF Edition)?

This app is funded with funds awarded from NHS England Staffordshire’s technology enabled care programme. If you have been diagnosed with atrial fibrillation (AF) and have questions about your condition, your GP or practice nurse can advise you further if you make an appointment at your practice.
