*2019-11-12, version 4.3*

I was given a card after my appointment in October.  Downloaded the app and it asks for user I’d and password.  Password was on the card but my user I’d isn’t.  Why not give me all the info I need to use the app?   Now I have to call the office and get it.  I hate superfluous steps.
