Your “choices” devolve to just preference of order. The choices for characters is your choice between an AD or no ad. All choices at a neighborhood level feel, even when choosing “to ignore” being pickpocketed, still results in police heat. Spend $30 USD and you get to skip basically 10 ads a day, not really worth it. Your only real customization is your choice between man/woman at the beginning, who leads the family when the don dies, and then the family name.
