MDHP Diabetes is here to create your personalized health journey with diabetes to keep it under control. This portal allows you to track your health anytime and anywhere just with a few clicks. By using a variety of measurement devices such as glucometers, blood pressure monitors, sports watches and activity bracelets, the app monitors this data to keep you in check. For some wearables, such as smartwatches and bands, workout and activity data can be taken by the app which communicates with Apple health for this data. 

During your journey, you will receive personal messages and feedback from your care team to help you better manage your health. Stay on track with notifications and alerts to monitor your progress on a regular basis. You will always be able to connect with your care team for any information and support. This allows for easy communication between you and your care team to ensure you are getting the best care you deserve.
