Fixes in this release:

- The appointment note was missing from the info card that opens when you tap an appointment in the calendar. The note is now shown there.
- Clearing an appointment note had no effect and the previous note remained. Notes can now be cleared.
- Patient notes written with formatting in the web panel appeared as raw markup in the app. They are now rendered properly.
- Generating a protocol number in patient details did nothing — no number and no error. It now works.
- When assigning a package to a patient, the package field came before the practitioner even though the package list depends on the practitioner. The order is fixed, and a package cannot be picked before a practitioner.
