Meridian Health is the patient app for Meridian GI and Metabolic Health, an upper GI and metabolic health practice in Fitzroy, Victoria. It is a quiet companion to your care between visits, built and maintained by the practice itself. The app is available to Meridian patients. Your care team will provide a patient ID and a one time setup code when you join. YOUR CARE JOURNEY Your care team sets up your journey in the app, including your procedure and key dates, so what you see always reflects your actual care plan. Follow your preparation tasks, know what to expect on the day, and work through your recovery phases with simple checklists. A Today card on the home screen shows where you are in your journey, your next task and your next appointment at a glance. RELIABLE INFORMATION Clear, practice written information about conditions, surgeries and procedures, perioperative care and common investigations such as gastroscopy, colonoscopy and body composition scans. Choose between basic and detailed reading depth. Your care team can also assign reading material directly to you. HEALTH TRACKERS Log weight, fluids, meals, exercise, medications, blood pressure, waist measurement, blood sugar and more. Estimate the nutrition in a meal from a photo. Your entries sync securely to the practice so your care team can see your progress and support you between appointments. STAY CONNECTED See appointments made by the practice alongside your own, with reminders. Set annual blood test reminders. Eligible programme members can message their care team securely within the app. PRIVATE BY DESIGN Your record stays inside the practice, where it should be. The app is held to clinical standards, not consumer ones. Appointments can now be edited or deleted after they are created, so your schedule always stays accurate. Fixed an issue where a newly assigned care journey could take too long to appear in the app. Contacting the practice is now easier: the enquiry email opens pre-addressed with a guided template for patients and referring GPs, so your message reaches the right person faster. A warmer, more refined visual design across the app, including softer background tones and a richer navigation menu.
