This app is going to make such a difference in my practice. I have already recommended it to everyone in my organization. In future updates I would love to see a demographic section, the ability to ‘uncheck’ yes/no boxes, and  ‘unknown’ option added as an option  to LMP date. Thank you so much for putting this tool together!
