## What is Mind Therapy Clinic?

Mind Therapy Clinic's patient app is a companion for people already receiving care at Mind Therapy Clinic. It gives you one place to stay on top of the between-session parts of your treatment — your schedule, your check-in questionnaires, and the paperwork your care team needs.

This app is for established Mind Therapy Clinic patients. What you can do:

• See your schedule. View your upcoming group and program sessions, times, locations, and whether a session is in person or online.

• Complete outcome questionnaires. Fill out the check-in measures your clinician assigns (such as the OQ-45.2 outcome questionnaire) directly in the app, so your care team can track how you're doing over time.

• Handle paperwork and requests. Submit and track requests to your care team — for example leave-of-absence notices, records-release (ROI) forms, prescription and enrollment requests — and sign documents when needed.

• Manage your profile. Review your contact details and, where available, subscribe to your session calendar.

The app keeps your information private: you're signed out automatically after a period of inactivity, and your screen is hidden in the app switcher when you leave the app.

This app is not for emergencies. If you are in crisis, call or text 988, or call 911.
