## What is My Kidney Team?

My Kidney Team is a personal organizer for people living with chronic kidney disease (CKD), a private place to log your numbers and learn general background information, in plain language.

IMPORTANT: This app is for organizing your own information and general education only. It does not diagnose, treat, cure, or prevent any disease, and it is not a substitute for professional medical care. Always talk with your doctor about your results and treatment.

ORGANIZE YOUR NUMBERS
• Log lab results by hand, or by taking a photo of a lab report to fill fields automatically
• See your entered creatinine converted to eGFR using the published CKD-EPI 2021 formula
• See where your numbers fall on the standard KDIGO reference chart (stages G1–G5, A1–A3) for reference only, not a diagnosis
• Trend charts to help you notice changes over time
• KDIGO risk reference chart, shown for general education

TRACK WHAT YOU MEASURE
• Blood pressure log, with the commonly cited CKD guideline range (less 130/80) shown for reference
• Weight log to help you notice changes over time
• A private symptom diary and optional PHQ-2/PHQ-9 mood check-in — self-reflection tools, not a diagnostic test
• Medication list with reminders you set yourself
