CareCognitics has made it easy for you to manage your long-term by connecting you to your doctor. If you doctor is participating in our program, through this app you can:
·         Get access to your care plan that contains your chronic conditions, your medication list, allergies that you may have and personal recommendations on how to manage your chronic conditions
·         Fax your latest care plan to any urgent care or specialist right from within the app
·         Timely reminders for critical appointments and overdue tests
·         Report any ER visits and hospitalization to your doctor for follow-ups
·         One-touch call to your doctor for quick access to the medical staff
 
Though CareCognitics does not recommend treatments to you, we help facilitate critical exchange of health information between you and your primary care doctor.
