Encompass Health inpatient rehabilitation patients and their designated caregivers have the option to use the MyEncompass Health Caregiver application to follow along with a patient’s rehabilitation program. The app shows a patient’s real-time progress toward their goals and an overview of their care plan in a secure manner. Information in the app is regularly updated by our care teams based on the individual care plan for the patient. TRACK Follow the patient’s stay at our hospital as they make progress toward established goals. - Track goals established by the patient and the hospitals interdisciplinary care team, including mobility, self-care, cognition and behavior, and daily living skills such as meal preparation and medication management. - See anticipated discharge date. - Plan for discharge, with instructions that may include: * Discharge diagnosis * Instructions for care from nursing and therapy * Follow-up appointments * Medications * Other pertinent information for care at home SHARE - Invite others to follow along as the patient progresses in their rehabilitation stay. - Allow the patient’s legal representative to share their information with others, through granting access to the application on their behalf. The dashboard of progress doesn’t seem accurate or up to date. I can’t even see the schedule for therapy. And now the past 2 days it says “application not available, try again later”. It’s a waste of phone memory. ### 1/5 — This app is a joke *2026-05-26, version 1.4.14* This app is completely unhelpful. I am at the encompass facility and cannot log in, and the staff cannot figure out what the issue is….so even the staff cannot set this useless app up. ### 1/5 — Useless *2026-04-22, version 1.4.14* It doesn’t actually tell you anything…. Quite frankly, the families of stroke patients don’t need this nonsense adding to their stress. Thanks for making things harder on me. Don’t bother!Care team cannot fix this & no helpdesk to call *2025-11-24, version 1.4.4* This app is supposed to allow the patient or proxy to share or invite up to 20 others to view minds p progress. It’s already been a 7 days and neither the nurses, case manager, nor admissions can figure out why this doesn’t work. There is no help desk to call, so the effort and aggravation has been useless. ### 2/5 — EHC *2025-09-18, version 1.4.4* The app is not updated or accurate. If something is 6/9 it’s not 100% even though it says 100%. Family and Friends can’t access the full report. Either fix it or remove it until it works fully. ### 3/5 — Ok on Android; errors on IOS *2025-02-05, version 1.4.2* IOS app only gives errors. Installed and runs on my Android phone. Can’t access via my iPad. Have re/installed a couple of times. ### 1/5 — Never Worked *2024-12-18, version 1.4.2* This app never worked for my father or myself. The tech support is inadequate and could never figure out what the issue was. They tried to blame our case manager. Worthless. I’d give no stars if it was an option. The program doesn’t work, should be removed from the AppStore, and cease advertising this piece of garbage!! It only has one star because I couldn’t remove it once selected.
