## What is MyHealthPort?

MyHealthPort offers a simple to use application to which allows you to video conference with your provider, as well as transmit measurements from connected diagnostic devices, such as:

• Glucose
• Blood Pressure
• Weight
• Temperature
• Blood Oxygen

Through the MyHealthPort platfom, the patient, members of the patient’s healthcare team, family, or friends, can receive access to historical diagnostic test results, as well as receive notifications based on rules set with their healthcare provider. You have full control of being able to submit and review complete and accurate test results, along with a safe and secure video conferencing solution. With MyHealthPort, you can better manage your condition and achieve your lifestyle goals.
